Accessing the services needed for the elderly is a difficult process that  can be a stressful mystery for families.  Often the elderly are left without because their loved ones simply do not know how to find the help they need.

 

As your Geriatric Care Manager, I will utilize my experience in Geriatric Social Services to provide the support you need for your loved one.

 

I will go to the home and or facility and perform an Assessment to identify needs and strengths and develop an Individualized Care Plan to meet those needs. I will work with the family, medical team and others to coordinate and advocate for appropriate services.

 

I have worked with Physicians, Attorneys, Bank Managers and VA Admnistration to help patients and/or clients.

 

I can assist caregivers to care for parents and loved ones in areas of Allegheny and Washington Counties.
